Computer systems failure

It seems many government computer systems don't deliver what they were supposed to or they are delivered way over budget. Over the last few years, the Child Support Agency and Passport Agency computer systems spring to mind. Have a look at a couple fo these BBC stories and discuss the points below.

In which stages of the systems life cycle are the failures?
What are the main reasons for computer systems not doing their job?
Do government computer systems fail more than those in the private sector? If so, why?

The article on Swanwick airport shows that there system is failing for a number of reasons. One of these is that the transmissions are falling out and that radio communications are being lost erratically. This means that there are long delays because of a computer failure. I think that all these problems should have been sorted so that this wouldnt happen. The reasons for this problem was that they hadnt future proofed the system so that they had to change it all because of the doubleing of air traffic. Also they have had different companies trying to create a system, and they had different ways of operating. http://news.bbc.co.uk/1/hi/uk/1993586.stm
